Errors by NHS staff led to 5,700 deaths in six months, report says

More than 5,700 patients died or suffered serious harm as a result of staff errors in the NHS over a six-month period, figures showed today.

Figures from the National Patient Safety Agency showed that 459,500 patient safety incidents and near misses occurred in England between last October and March.

This is a 12 per cent increase on the previous six months but the agency, which collects and monitors safety data in the NHS, said that better reporting was fuelling the rise.

Palestinians urge world: Stop Judaization of Jerusalem

The Western-backed Palestinian Authority urged the international community on Monday to "force it [Israel] to put off its attempts to take over Jerusalem and Judaize it".

The Palestinian cabinet, issuing a strong statement after a meeting in the West Bank town of Ramallah, pledged "to confront Israel" as Israel Defense Forces troops clashed with protesters for a second day in the Jerusalem area.

Palestinian youths hurled rocks at Israel policemen and burned cardboard cartons and trash in the streets of Shoafat in Arab East Jerusalem, after Israel arrested a teenager it suspects stabbed and wounded a soldier conducting a security check on a bus.

The violence spread to the outskirts of Ramallah, where about 50 Palestinian teenagers took cover behind trucks and cars while hurling rocks at Israeli soldiers, who reporters saw responding by firing tear gas and rubber bullets.

One policeman was wounded by a rock and seven protesters were arrested. Nine Palestinians and two Israeli policemen were treated for minor injuries in scuffles that erupted in Jerusalem on Sunday, and 30 were hurt in similar clashes a week ago.

Palestinians have warned that the tensions flaring over access to a holy compound housing the al-Aqsa mosque, an area also revered by Jews as the site of an ancient temple, could, on the background of stalled peace talks, ignite a third uprising.

We Need a Bigger House
The remedy for an undemocratic system: more democracy.

Watching the House of Representatives on late-night C-SPAN, you might have any number of reactions, including seppuku-inducing boredom. Depending on who’s talking, you might also feel disgust, rage, contempt, or, in rare cases, inspiration. But one reaction you probably won’t have is: “Gosh, if only there were more of these jokers.”

That’s too bad. Because what our political system may be lacking more than anything else is enough members of Congress. No, really. Seriously, stop laughing.

Except for a brief effort to accommodate Alaska and Hawaii, the size of the House has been frozen at 435 members since 1911. A 1929 law, driven in part by a desire to keep immigrants underrepresented, has kept it that way.

But there’s nothing sacred about the 435 number. In fact, the Founders would be aghast at the idea that the “peoples’ house” is filled with pols speaking for hundreds of thousands of citizens.

... Under today’s apportionment system, the “ideal” congressional district is 700,000 people, with some districts reaching nearly 1 million. Montana, with a population of 958,000, has just one representative, but each of Rhode Island’s two districts has about 530,000 people.

There is, of course, an important principle here, and if all of Montana’s residents were black, it would be easier for everyone to see it. Montanans’ votes don’t count as much as Rhode Islanders’ — in fact, a Montanan’s vote only counts for about three-fifths of a Rhode Islander’s. That America’s slave population was counted by the same ratio under the original Constitution is usually cited, rightly, as one of the document’s greatest sins. A lawsuit filed in federal court in Mississippi last month hopes to force Congress to remedy the status quo’s assault on the one-person, one-vote principle by increasing Congress to as many as a paltry 1,761 members.

A SIMPLE IDEA TO MAKE AMERICA'S GOVERNMENT EVEN MORE REPRESENTATIVE AND RESPONSIVE

Glenn links to a review of the new Larry Sabato book which recommends 23 - YUP TWENTY-THREE! - amendments to the Constitution.

One wonders why Sabato doesn't just throw it out - like Chavez - and start from scratch!

I have a simple idea which would improve both the responsiveness and representativeness of the federal government, and it would NOT require a single solitary change to the Constitution!

I would INCREASE the number of Representatives in the House to 600.

I would assign the new seats by using the US DECENNIAL CENSUS figures - exactly the purpose for which the Census was designed and put into the Constitution by the Framers.

These new seats would not be attached to an incumbent of any party and would therefore be open seats more readily contested fairly - (since incumbents have great electoral advantages and are returned to office so frequently).

Having more Representatives would make each district smaller and reduce the committee responsibilities of each representative and thereby allow her/him to do more constituent work - BE MORE RESPONSIVE.

The number of representatives was capped -BY STATUTE - in 1929:

The Permanent Apportionment Act of 1929 capped the Membership at that level, creating a procedure for reapportioning state delegations in the House under “the then existing number of Representatives” (see Act of June 18, 1929, ch. 28, 46 Stat 21).

It can be undone by statute. A simple majority vote by each body of Congress and a signature by the POTUS.

A body with 600 members is much more manageable now than a body with 420 in 1929. Many representative bodies in the USA and around the world have 600 members and function fine.

This change would also have the added benefit of making the electoral college more representative and more accurately reflect the wishes of the people.

DECENNIAL RE-APPORTIONMENT AND THE CENSUS

POLIBLOGGER posted on an article by FRUITS AND VOTES. F&V suggested we expand the House of Representatives and make it more representative and responsive and districts more equal in population size by using "THE WYOMING RULE": Under the ‘Wyoming Rule,’ the standard Representative-to-population ratio would be that of the smallest entitled unit–i.e. currently Wyoming. (Poliblogger rans the numbers.) I AGREE: we need MORE Reps. But maybe not that many. Here're my thoughts:

(1) The US CENSUS was specifically designed to allow for the ORDERLY EXPANSION of the House so that districts would be equal in size, and the number of districts would expand as the population expanded.

(2) The problem is that since 1920, the number of districts has been frozen at 435. This was done by an ACT OF CONGRESS; it can be undone by an ACT OF CONGRESS, too. It doesn’t require a constitutional amendment.

(3) Since 1920, every decennial census has led MERELY TO RE-APPORTIONMENT: changes/shifts of Reps withing the 435. Some states get more Representatives other states get theirs reduced. Sometimes a state which has grown in population may have their number or Reps reduced because all states must have at least one Rep.

(4) The total number of Reps in the House could be and should be expanded - if not by the "Wyoming Plan" how about just increasing the number od reps in the House to a NICE ROUND NUMBER: 500 - by adding an additional 65 seats. (I think that 500 has a nice ring to it - especially for a nation of 50 states!) These could be/would be apportioned by population. Every ten years we dicide the population by 500 and distribute the seats accordingly.

(5) Average district size would be each smaller - and more representative, AS THE FOUNDERS ORIGINALLY PLANNED. More Reps would mean fewer committee assignments for each Rep and more constituent time. (Also, the advent and pervasiveness of hi-tech communications makes the management of a larger House less encumbering than in 1920, or since 1920. In fact, MANY national and state legislatures all over the world have many more mebers than we do and are larger than a mere 435. If they can mange it, then so can we.)

(6) As these 65 NEW SEATS would ALL BE OPEN SEATS, they’d be VERY competitive races at a time when most seats are NOT COMPETITIVE. That’s a good thing too. This would be A NEW BIRTH OF REPRESENTATION, which would NOT make government bigger, but make it MORE RESPONSIVE AND MORE REPRESENTATIVE.

ALSO: (7) This would change the ELECTORAL COLLEGE - making it even more reflective of the population.

I DARE A BRAVE CONGRESSMAN TO INTRODUCE THIS. It should get BI-PARTISAN SUPPORT, becasue open seats are VERY COMPETITIVE, and the GOP is VERY COMPETITIVE in the CITIES AND COUNTIES which have had the most growth.

My gut feeling is that the present recovery from the financial abyss will now be followed by a secondary downturn as the echoes of the first shock work thru the huge pond of liquidity the Central Banks have poured into the system.

All that liquidity was soaked up by the banking system and very little made it out to the real economy. Companies that did not immediately adjust to the crisis are now firing staff and changing course. If the consumer doesn't wake-up this holiday season it's gonna get real ugly.

Opec wants to get out of the dollar - or at least reduce its dominant role in oil trade.

China is holding too many dollars for their own good.

Japan likes the dollar as they depend on US protection.

The Arabs, China, Russia all want to weaken the USA.

Where do India and Brazil fit in?

Remember: a weaker $ does not necessarily = weak USA

A weak President does = a weak USA

In many areas of concern President Obama does not seem committed to any course of action - except to preserve his power and stay loyal to his own empty rhetoric.

More on the ocean acidity scare

There is quite a debate over ocean acidity going on among the skeptical community and I seem to get most of the emails copied to me. The debate is over just how ocean acidity works. The Warmists just assume they know but the facts contradict their ideas. I have no claims to being a chemist, however, so I am not going to post any more of the debate. I put up two excerpts yesterday so from now on I will wait until someone else who really knows the science puts it all together. Meanwhile, however, a reader has sent me some links that look very interesting:

See this PDF of pH measurements of "Monterey Bay Aquarium Incoming Seawater". Note that from when measurements started until the present, there is no trend. The range is from about 7.7 to 8.1, with the first and last being about 7.9, which is consistent with this PDF -- which makes clear that ocean pH is highly variable, with an "average" of around 7.6 (a skewed distribution?), meaning that the bay is more alkaline than average ocean water.

So any claim that there is a "given", global or proper level of ocean acidity is as absurd as the claim that there is a "given", global or proper level of temperature. Acidity is highly variable, follows no simple rule and we have much to learn about it. Warmist certainty in the matter is just the work of con-men with no respect for the facts.

Why are webhosts so ill-mannered?

The internet and books have opposite vices and virtues. Books are highly permanent (we have some copies from a couple of thousand years ago) but are not readily accessible. You generally have to take a trip to a library if you want to access one. The net on the other hand is instantly and very conveniently accessible but it is also evanescent. A file of writings or a picture on the net can disappear in an instant and maybe never be found again. And the vanishing act is often the fault of the webhost rather than anything that the writer or photographer concerned has done. Many newspaper sites, for instance, seem to have a policy of deleting their own articles after quite short periods of time.

But I like to keep all the files of my writings publicly available regardless of what any individual webhoster or bloghoster might do. So I try to address the big weakness of the net by following the systems theory axiom that redundancy is the path to systems reliability. In other words, I keep multiple copies of my files up -- so that if one copy disappears, there will be others to be found. As a result, I have had a lot of dealings with the various webhosts where I place copies of my files/writings. And it is truly amazing how many webhosts I have gone through over time. Whether "free" or paid, there are at least a dozen (maybe 20) locations where my files were once found which no longer host my files. Some webhosts have simply gone bust and disappeared altogether and others have decided that my files are "incorrect" in some way and have deleted or blocked them.

And I am not complaining about that. It is precisely because I expect such impermanence that I keep multiple copies of my files online. What I DO object to, however, is that NOT ONCE have the webhosts concerned had the manners to email me in advance and warn me or consult me about what is going to happen. It is always a case of "shoot first; ask questions later".

It is actually a rather common event for webhosts to be "down" for various periods -- sometimes for a week or more. One would think that on such occasions, they would email their users and say something like: "We are having problems. Don't go away. We expect to be back up in a couple of days". But that never happens. The site eventually comes back online with no explanation or apology. Even sites that boast that they talk to their users are the same. And some sites of course NEVER come back up and you are left to figure that situation out. Even if you email them to ask if the cessation of service is temporary or permanent, you never get a reply.

And the Blogspot subsidiary of Google (which hosts this blog) is as bad as any of them. You would think that a big company like Google would be conscious of PR but it is not so. They too act first and ask questions later. I have lost track of how many times my blog sites have been blocked by them -- and never after any advance warning. Usually, I can fill out a form, the form gets acted on, and the site is unblocked but NEVER have I received an explanation as to why the block was put on. And sometimes the block lasts for over a week: At which point I tend to move to another bloghost, which is part of the reason why I now have two blogs hosted on Wordpress and two on blognow.com.au. That comes at some cost as I lose my old page ranking, so if anyone reading this has a site of their own it would help if they put up a link to my new POLITICAL CORRECTNESS WATCH site.

And sometimes filling out the Google/Blogspot form does you no good at all. The obstruction is permanent and repeated requests for review of the block are ignored -- again without explanation. No manners or anything else much there.

I have become VERY wary about such shenanigans over the years, however. I don't blame anybody for thinking that I have gone overboard but at the moment I have my files spread over ELEVEN different webhosts. None have all my files but between them all there are usually three copies of any file available. I realize that I must sound slightly mad but I think that if you had had my experiences with webhosts you might be nearly as bad or mad. The way I have it set up, if one host goes down, I have to reload only a subset of my files, not the whole lot. Even that can be pretty pesky, though. And I have various pages -- including this one -- which have sufficent links to lead you to ALL my files (writings), regardless of where they are hosted.

So what is the answer to the question I ask in the heading above? I can only see the answer as lying with the general lack of civility these days. Under the influence of the prevailing Leftist gospel that "There is no such thing as right and wrong", people who can hide behind anonymity see no reason for civility. Maybe there is a Christian webhost somewhere who has higher standards than that but I have yet to find one.

Despite all the noise about this recent failure, we do need to remember that BHO's Olympic fail is not an impeachable offense.

US officials are expected to announce plans that would allow illegal immigrants not considered a threat to be taken out of jails, reports say.

The new policy would list immigrants according to the risk they may pose, the Wall Street Journal reports.

Detainees who are not criminals could be kept in hotels and nursing homes, according to leaks of the plans.

Homeland Security Secretary Janet Napolitano is expected to give details of the plans later.

Her department is hoping to cut the costs of detaining immigrants, which stood at almost $2bn (£1.3bn) in 2008.

It says alternatives like the hotels and nursing homes would cost about $14 a day, compared to about $100 a day for detention.

RIOTS AT TEMPLE MOUNT AND SHUAFAT

In the past 2 days, there were Muslim riots in Jerusalem circa the Temple Mount and the Arabic neighborhood of Shuafat, planned by the "peace partner" that goes by the name of Fatah (via Israel Matzav and Rick's Cafe American):

Police revealed Monday that officers had discovered wheelbarrows filled with bricks and heavy rocks on the Temple Mount on Sunday. The rocks were apparently prepared by Muslims who planned to use them against police and Jewish worshippers in case of a riot.

The discovery was one of the factors that led to the closure of the Temple Mount to Jews and to limited entrance to Muslim worshippers, police said.

Another factor was ongoing incitement from Muslim leaders, several of whom have accused Israeli Jews of planning to harm the Al-Aksa Mosque on the Temple Mount. On Sunday and Monday, Muslim leaders in Jerusalem and around the country called on young Muslims to “come and protect Al-Aksa.”

The call to protect the mosque from alleged Zionist plots spread quickly, and on Monday thousands of Muslims in Gaza, Judea and Samaria took part in solidarity rallies calling to protect Muslim control of the Temple Mount compound.

Between the incitement and the prepared rocks, police came to believe that rioters were planning more trouble, and that previous riots were not merely a spontaneous response to the presence of religious Jews on the Temple Mount, as Muslim worshippers had previously claimed.

And appallingly enough, we cannot expect any UN, Obama administration or EU officials to care. Come to think of it, we can't even expect any proper action back at home.

The riots are still continuing, and one of the masterminds is Ra'ed Salah (also via Rick's Cafe American and Yaakov Lozowick):

Salah told Haaretz on Monday that the clashes would last as long as Israel's "occupation" of the city and Al-Aqsa Mosque continued. He said the Israeli government must understand that using force does not grant it rights to Al-Aqsa Mosque or anywhere else in East Jerusalem, and that the key to achieving calm in the area is an Israeli "withdrawal." "No one has rights to the Al-Aqsa Mosque other than the Muslims. The mosque compound is Muslim, Palestinian and Arab, and Israel has no rights to the mosque or East Jerusalem," he said. Salah has been prohibited from entering the Temple Mount area for several months, and has been staying at a nearby residence while following developments. The Islamic Movement leader reiterated his call for Arabs within Israel's Green Line and in Jerusalem to protest beside the mount to "protect Al-Aqsa from the infiltration of extremist Jewish elements."

That man is simply dangerous, and should be banned from Jerusalem altogether for the incitement he's committing. Update: as a matter of fact, he now has been, but only for a month:

Sheikh Raed Salah, the leader of the Islamic Movement's northern branch, was banned from Jerusalem for 30 days on Tuesday after being arrested on suspicion of inciting the recent Arab riots in the capital.

The banning order was issued by the Jerusalem Magistrate's Court.

Salah was arrested in the Wadi Joz area of east Jerusalem, on a rooftop of a home where members of the movement's northern branch had congregated.

It isn't the first time Salah has landed himself in trouble with Israeli authorities.

In March this year, Jerusalem police arrested the fiery leader for assaulting police officers at an illegal east Jerusalem gathering.

In January 2008, Attorney-General Menahem Mazuz filed an indictment against Salah, charging him with incitement to violence and racism in a speech he made protesting the archaeological dig carried out at the Old City's Mughrabi Gate. During his sermon in Wadi Joz in February 2007, Salah had urged supporters to start a third intifada in order to "save al-Aksa Mosque, free Jerusalem and end the occupation."

In addition, Salah served a two-year jail sentence starting in May 2003 for a series of security offenses, including financing Hamas activities.

Well in that case, he should be imprisoned for life! He's been bankrolling terrorist gangs.

RIFQA BARY'S BROTHER BRAGGED THAT HE GAVE HER FOSTER CARE LOCATION TO MUSLIM BLOGGER STALKER

The Jawa Report has some very disturbing news: a source who knows Bary's family told them that her brother, Rilvan, repeatedly bragged about giving the info on her foster care location to a Muslim writer for the Examiner in San Francisco named Davi Barker. The source also added that Rilvan provided Barker with info about Rifqa's Christian associates in Ohio. Whether or not this is so, if he did do this, then the brother has committed a potentially criminal action, and come to think of it, Barker may have too. I should hope that this is going to be addressed in the further coming court hearings, and the brother will have to be banned from meeting with her again.

FTC to Regulate Blogging‏

This sounds very much like the thin end of the wedge. What will they try to regulate next?

"The Federal Trade Commission will try to regulate blogging for the first time, requiring writers on the Web to clearly disclose any freebies or payments they get from companies for reviewing their products. The FTC said Monday its commissioners voted 4-0 to approve the final Web guidelines, which had been expected. Violating the rules, which take effect Dec. 1, could bring fines up to $11,000 per violation. Bloggers or advertisers also could face injunctions and be ordered to reimburse consumers for financial losses stemming from inappropriate product reviews.

The commission stopped short of specifying how bloggers must disclose conflicts of interest. Rich Cleland, assistant director of the FTC's advertising practices division, said the disclosure must be "clear and conspicuous," no matter what form it will take...

The FTC's proposal made many bloggers anxious. They said the scrutiny would make them nervous about posting even innocent comments. To placate such fears, Cleland said the FTC will more likely go after an advertiser instead of a blogger for violations. The exception would be a blogger who runs a "substantial" operation that violates FTC rules and already received a warning, he said.

Barack Obama furious at General Stanley McChrystal speech on Afghanistan

The relationship between President Barack Obama and the commander of Nato forces in Afghanistan has been put under severe strain by Gen Stanley McChrystal's comments on strategy for the war. According to sources close to the administration, Gen McChrystal shocked and angered presidential advisers with the bluntness of a speech given in London last week.

The next day he was summoned to an awkward 25-minute face-to-face meeting on board Air Force One on the tarmac in Copenhagen, where the president had arrived to tout Chicago's unsuccessful Olympic bid.

Gen James Jones, the national security adviser, yesterday did little to allay the impression the meeting had been awkward. Asked if the president had told the general to tone down his remarks, he told CBS: "I wasn't there so I can't answer that question. But it was an opportunity for them to get to know each other a little bit better. I am sure they exchanged direct views." An adviser to the administration said: "People aren't sure whether McChrystal is being naïve or an upstart. To my mind he doesn't seem ready for this Washington hard-ball and is just speaking his mind too plainly."

In London, Gen McChrystal, who heads the 68,000 US troops in Afghanistan as well as the 100,000 Nato forces, flatly rejected proposals to switch to a strategy more reliant on drone missile strikes and special forces operations against al-Qaeda. He told the Institute of International and Strategic Studies that the formula, which is favoured by Vice-President Joe Biden, would lead to "Chaos-istan". When asked whether he would support it, he said: "The short answer is: No." He went on to say: "Waiting does not prolong a favorable outcome. This effort will not remain winnable indefinitely, and nor will public support." The remarks have been seen by some in the Obama administration as a barbed reference to the slow pace of debate within the White House.

Gen McChrystal delivered a report on Afghanistan requested by the president on Aug 31, but Mr Obama held only his second "principals meeting" on the issue last week. He will hold at least one more this week, but a decision on how far to follow Gen McChrystal's recommendation to send 40,000 more US troops will not be made for several weeks. A military expert said: "They still have working relationship but all in all it's not great for now." Some commentators regarded the general's London comments as verging on insubordination.

IDF Chief of General Staff Lt.-Gen. Gabi Ashkenazi flew secretly to France on Sunday for meetings with Adm. Michael Mullen, the chairman of the US Joint Chiefs of Staff, and Gen. Jean-Louis Georgelin, the chief of the French Defense Staff.

Ashkenazi left Israel early on Sunday morning and flew to Normandy, where he met with Mullen. The two have developed a close relationship since Ashkenazi was appointed chief of General Staff in 2007 and they reportedly speak at least once a week by phone.

... The meeting in Normandy focused on assessments regarding Iran, Syria, Hizbullah and the joint Israeli-US Juniper Cobra missile defense exercise that is scheduled to start later this month. The exercise will include the Israeli Arrow missile defense system as well as three American systems - the THAAD, Aegis and PAC3 - that will all be deployed in Israel for the duration of the exercise.

Obama: The World's Evil Clown

Appeasing the mullahs, propping up a dictator in Honduras, taking the side of barbarity and terrorism in the Middle East, throwing Eastern Europe under the bus, and generally supporting evil while shunning our allies. And that's just his foreign policy. This corrupt, immoral, unethical clown needs to be removed from office.

President Obama's first eight months on the world stage are deeply troubling. It is not just the repeated apologies for alleged American transgressions. We are witnessing a disturbing pattern that extends a hand of cooperation to our enemies while turning a cold shoulder to our friends and a blind eye to their suffering.

It was evident in the streets of Iran, where dissidents rallied for the right to have their votes counted and their voices heard. Their cries for a free and fair election and basic human rights were written in English intended for a watching world. The Iranian government responded with a crackdown on the dissidents that turned deadly.

The week before violence broke out in Iran, President Obama spoke eloquently about the fundamental rights of all individuals to have a say in how they are governed. The conventional wisdom and the global anxiety regarding an Israeli attack on Iran is totally wrong.

If Israel attacks Iran there will be no major conflagration as a result; oil will not hit $400/barrel.

The weenies and fear-mongers said the same thing in 1991 regarding the Gulf War and said the same thing before the Iraq War.

When Israel attacked Syria and destroyed its nuclear assets nothing happened.

There was no Muslim uprising or Iranian counter-attack - despite there being a mutual defense pact between Syria and Iran.

Nothing will happen this time.

Saudi Arabia and all the Gulf states want Israel to attack.

Only the jihadists and the cowards and appeasers of the left-wing - who have been the jihadist's Fifth Column and virtual comrades - think there will be a major backlash.

• THE LONGER WE WAIT THE STRONGER IRAN AND ITS STOOGES BECOME AND THE WORSE WILL BE THE ULTIMATE CONFLICT.
